Individuals who have made the decision to remain at home rather than in a facility, and who want expert nursing support to do so safely and comfortably.
Families enrolled in hospice who need additional skilled nursing hours or hands-on support beyond what the hospice team provides.
Patients discharged from the hospital with a comfort-care or “no further treatment” plan who wish to spend their remaining time at home.
